Intercommunity Health Network Coordinated Care Organization Delivery Systems Transformation (DST) Symposium
Faith Community Health Network Board members were on hand at the Intercommunity Health Network Coordinated Care Organization Delivery Systems Transformation (DST) Symposium on October 1, 2026 to present the impact of the DST-funded pilot project called Improving Senior and Disabled Access to Care. Tawni Pfaff, BSN, RN, Faith Community Nurse, serves pro bono at a local senior apartment complex for just two hours, twice a month. Simply by being available to answer resident questions, she has been able to avert several emergency response activations and potential hospitalizations. She offers spiritual support and connects residents to community resources. This has been an exciting project with surprising results!
Angel Harris, MSN, RN, Faith Community Nurse/FCHN Board Education & Event Coordinator, Deb Fell-Carlson, BSN, RN, MSPH, Faith Community Nurse/FCHN Executive Director, and Liz Nation, RN, Faith Community Nurse/FCHN Board Treasurer engaged symposium participants in conversations about the FCHN “Improving Senior and Disabled Access to Care” pilot and spent time networking with other participants.
Delivery Systems Transformation Committee co-leads Michael Couch, Executive Director of Crossroads Communities and Alisha DeVogele, Health Equity Program Manager II with Samaritan Health Plans update attendees on the funding status of recent proposals submitted through the DST.
